Please arrive 10-15 minutes early for check-in and provide previous medical records.

If urgent, please call us ASAP. Otherwise, please enter notes about illness and urgency.

Drop off is between 7:30am and 8:30am the morning of the appointment. No food after 8:00pm the night before the appointment. Water is okay.
